- Private Applicator Permit Application
- The Private Applicator Permit Application is sent to
Private Applicators with expiring permits from OISC. It is also the form to
complete for a Reciprocal Private Applicator Permit.

Exam Brochure
- The tri-fold
brochure lists the dates and locations of the pesticide core exam for private
applicators. Includes registration form needed for taking the exam at the
regional sites.

Sheet: Private Applicator Number-only
- Private Applicators with their PA permit number sign
this sheet to receive credit for a pesticide recertification program.

- Attendance
Sheet: Individual SS#-only Registration
- A Private Applicator can complete this individual form
with the last 4 digits of their Social Security Number if their PA permit
number is not known, in order to receive recertification credit at a program.

website and create mailing labels in Word.

- Recertification
Program Approval
- Program must be
submitted no less than 2 weeks in advance to Cheri Janssen.
Include
date, time, location, topic(s) presented, length of presentation and contact.
Submit via mail, fax, email (not by phone).
It's helpful to
include on the website and program phone line if advanced registration
is required or fees other than the $10 will be charged.
Approval will be
given within one week and a program number will be assigned.
