Photo Exempt ID
A government-issued photo identification is required at all pesticide exam sites. However a government-issued photo exempt identification card from the Indiana Bureau of Motor Vehicles will also be accepted on grounds of religious beliefs.
The ID card can be obtained at local Bureau of Motor Vehicle offices (License Branch).
The Bureau of Motor Vehicles requires specific documentation when obtaining any license/permit for the first time. You can see the list of required documentation at the following website:
